NDIS services

How we support you

At Care Managers, our NDIS services are designed to make everyday life more manageable, more flexible, and more tailored to your goals.

Practical, flexible, and centred on you

NDIS services built around real needs

Everyone’s experience with the NDIS is different — and so is the kind of support they need. That’s why our NDIS services are designed to be flexible, practical, and shaped around how you want to live.

From daily personal activities to transport and community access, our team works closely with you to build a support plan that reflects your routines, your preferences, and your goals. You don’t need to have all the answers before you start — we’ll help you figure out what’s possible and how we can make things easier.

Whether you need short-term assistance or long-term support, Care Managers is here to help make your plan work for you.

Community Access

Community Participation

We support you to take part in everyday activities that help you feel connected and confident in your local area. That could mean going to appointments, heading to the shops, joining in on community events, or simply getting outside for a walk and some conversation.

We plan around what matters to you — your routines, your comfort levels, and your interests. You’ll always be supported by someone familiar, friendly, and respectful. Whether it’s something social or something practical, our aim is to make it easier for you to stay involved in your community and feel good while doing it.

Innovative Community Participation

This service is all about exploring new opportunities in a way that feels engaging and meaningful. Whether you’re curious about trying a new hobby, joining a local class, volunteering, or discovering creative ways to meet others, we’ll support you to find what suits you best.

It’s not about sticking to a standard program — it’s about doing something that sparks interest and adds value to your week. We take the time to understand what you’re looking for and help you build skills, confidence, and social connections along the way. You set the direction — we’ll support the journey.

Group and Centre Based Activities

As part of our NDIS services, we offer group and centre-based activities that give you a chance to socialise, learn, and enjoy your time in a relaxed setting. Whether you’re into music, arts and crafts, games, cooking, or themed events, we tailor our programs to suit a range of interests and support needs.

These activities can help build confidence, improve communication skills, and create a genuine sense of connection with others. You can choose how often you join in and what feels right for you. It’s a welcoming space where participation is encouraged — and always supported.

Assistance with Travel/Transport

Transport can be a major hurdle — and we’re here to make it easier. We provide reliable and safe travel support to help you get where you need to go.

Whether it’s a medical appointment, a community event, shopping, or just catching up with friends, we’ll organise transport that fits your routine.

Our team is punctual, friendly, and trained to assist with mobility needs where required. You’ll be supported by someone familiar, so the experience feels comfortable and stress-free. It’s one less thing to worry about — and one more way we help you stay connected.

Supported Independent Living

Our Supported Independent Living NDIS services are designed to help you live as independently as possible, while still having access to the right support. Whether you live on your own or share with others, we’ll work with you to build a routine that supports your goals and preferences.

We can help with things like personal care, cooking, cleaning, and medication management. Our team will always respect your space, your choices, and your pace. With SIL, the aim is to support your independence while giving you the confidence and comfort to live your life the way you want to.

Daily Personal Activities

We provide assistance with everyday personal tasks that help you feel your best and stay comfortable at home. This can include support with showering, dressing, grooming, toileting, and meal preparation — always delivered with dignity and care.

Everyone has their own way of doing things, so we’ll take the time to understand what works best for you and adapt our support accordingly. Whether you need help every day or just occasionally, we’re here to make life that little bit easier. Our goal is to give you the support you need to feel confident in your own space.

High Intensity Daily Personal Activities

Some individuals require more complex or intensive care, and our team is fully trained to provide it. We offer support for high intensity personal activities such as catheter care, bowel management, PEG feeding, wound care, and medication administration.

These tasks require skill, attentiveness, and sensitivity — and that’s exactly what you can expect from our staff. We understand that this type of care involves a high level of trust, so we prioritise open communication and consistency. With our high intensity supports, you’ll be in safe hands, and we’ll work closely with you to make sure you feel supported and respected at all times.

Household Tasks

A clean and organised space can make a big difference to how you feel day to day. We can help with a range of household tasks including general cleaning, laundry, vacuuming, dishwashing, and light meal prep.

Whether you need help every week or just from time to time, we’ll step in where needed and support you to maintain a comfortable and safe living environment.

Our team works alongside you — not over you — and we’re always respectful of your home and your routines. This service is about taking the pressure off while helping you stay in control of your space.

Support Coordination

Understanding and using your NDIS plan can sometimes feel overwhelming — but you don’t have to figure it all out on your own. Our Support Coordination NDIS services help you connect with the right providers, understand your options, and make informed choices about how to use your funding.

We’ll explain things in a clear, straightforward way and support you to build a strong, reliable support network. Our focus is always on your goals and preferences, not just what’s available. With Care Managers, you’ll have someone in your corner helping you make the most of your NDIS plan without the confusion.

Daily Living and Life Skills

Building confidence in everyday life takes time, support, and the right opportunities. We work alongside you to develop practical skills that support your independence — whether that’s cooking meals, managing money, catching public transport, or organising your day.

These skills aren’t taught in a one-size-fits-all way; we adapt our support to suit your pace, interests, and lifestyle. Whether you’re working towards a specific goal or just want to feel more capable in your routine, we’re here to guide and encourage you. With the right support, everyday tasks can feel more manageable — and even enjoyable.
